What to wear at your therapy session?

Wear comfortable, stretchy or loose work-out type clothing that allows the therapist to access the area being treated. For example, if you have a knee injury, shorts would be ideal. If you have a shoulder injury, perhaps a loose shirt. You are welcome to change into comfortable clothing here.

What should I bring to my appointment?

Please complete our New Patient Form that includes your medical history and a Questionnaire for the body area that most troubles you – Shoulder, Upper Extremity, Cervical-Neck, Lower Extremity, Low Back.  The New Patient Form and all Questionnaires are located under Patient Info\New Patient Forms.
